Annual Notices of Change (ANOCs)
CMS has released final 2008 Annual Notices of Change (ANOCs) that Medicare Advantage and Part D plans must get to their 2007 members no later than October 31, 2007. The ANOC explains any plan structure, formulary and/or cost structure changes that the plan will implement in 2008. The ANOC is the starting point for the process of comparing and deciding about plan choices that every Medicare beneficiary should undertake this fall before the Annual Enrollment Period begins on November 15, 2007.

Evidence of Coverage (EOCs)
The Evidence of Coverage (EOC) is a very important document for all Part D members and Medicare Advantage plan members. The EOC is a lengthy document that lays out the actual terms of coverage under the plan. Plans must assure that 2008 members receive the 2008 Evidence of Coverage no later than the end of January 2008. Part D and Medicare Advantage plans are permitted by CMS to send a combined Evidence of Coverage-Annual Notice of Change (ANOC). Remember that the 2008 ANOC must be received no later than the end of October 2007.

Model Formulary Disclosures

CMS has released a template pharmacy directory (63Kb Word file) for Part D plans to use.

Model Transition Notice

CMS has also released the model transition notice (34Kb Word file) that Part D plans must mail to new members within three days of when they fill a prescription for a medication that is not on the plan formulary or that is subject to utilization management requirements, such as prior approval or step therapy.
